How it works

Making Positive Change will help you to you to discover important healthy lifestyle and make changes to your habits.

Our programme is designed for people who could benefit from lifestyle changes in order to reduce risk factors associated with cardiovascular disease.

You will work one-to-one with a personal Health & Wellbeing Coach to identify your own needs and decide what the right strategy is for you.

We have a range of topics of support available, including building healthy habits, mindful eating, sleeping better, self-confidence, managing stress, personal resilience and improving proactivity.

Our support includes a range of behaviour change techniques and tools so you can easily understand and apply what you learn to your daily life.

  • One-to-one sessions with a Health & Wellbeing Coach

  • In person or online

  • 15/30 minutes session duration Weeks one to six: 30 minutes, weeks seven to ten: 15 minutes

  • One session per week for ten weeks

    Get Started

    Introducing the programme, establishing your goals, completing an initial assessment and discovering what areas you would like support with.

  • Sessions 2-6:
    Core Topics

    Support with your chosen behavioural change topics matched to your own needs and goals.

  • Session 7-10:
    Check-Ins

    Tracking your progress and helping you access further support where needed.
